Philadelphia Business Journal: Holy Redeemer delves deeper into digital health

The Montgomery County provider is expanding its relationship with Chicago-based Prepared Health and the company’s enTouch network.

For the past year Holy Redeemer has used enTouch with its home care unit that operates in Pennsylvania and New Jersey. The enTouch platform is designed to provide caregivers and case managers with the ability to collaborate in real-time to eliminate communication gaps that occur when multiple providers are involved with patient care after a patient is discharged from the hospital. The platform also alerts parties to care updates as they happen.

Under the larger partnership, the platform will be expanded to allow Holy Redeemer to use enTouch technology to connect personnel in its hospital, life care centers and home care agency.

Patricia O’Brien, senior vice president and chief administrative officer for Holy Redeemer Home Care & Hospice, said Prepared Health’s technology supports the health system's commitment to safe transitions for hospice and home care patients and open, consistent communication with its referral sources and physician partners. “This technology allows our team to refer and accept patients, update our care partners on patient status, and work collaboratively to prevent unnecessary ER visits and hospital admissions,” O’Brien said.  

Ashish Shah, president and CEO of Prepared Health, said as patients move among the various care setting, communication among various providers has proven to be a “disjointed” process. “A patient can go into the hospital for a knee replacement and, after being discharged, may need to spend a short time in a rehabilitation facility and after that may need home health care,” Shah said. “EnTouch was designed to make it easy for case managers and care providers to stay connected.”

Holy Redeemer also will have access to DINA, Prepared Health’s Digital Nursing Assistant, which analyzes patient data and uses artificial intelligence to identify and recommend evidence-based interventions and improve post-acute transitions.

Tim Coulter, COO of Prepared Health, said the company’s mission to reduce avoidable hospital re-admissions and “drive better patient-centric data-driven experiences” for patients as they move from a hospital back to their home.  

Other Philadelphia-area customers for Prepared Health, which charges users a monthly subscription fee, include South Jersey-based home health care provider Bayada and post-acute care facility operator Genesis HealthCare of Kennett Square.

Shah said Holy Redeemer is its first Philadelphia-area health system client. “We will add more and we are working on it,” he said.
